Political Roles Influencing Changes in Public Health Administration in Loei Province

Abstract
This study aimed to examine (1) the decentralization of public health administration in Loei Province, (2) the political roles influencing changes in public health administration in the province, and (3) the impacts of political roles on public health management. A qualitative research approach was employed, combining document analysis and in-depth interviews with key stakeholders, including executives of the Provincial Administrative Organization, the Provincial Public Health Doctor, district public health officers, directors of sub-district health-promoting hospitals, and local public health officials.
The findings revealed that the transfer of public health functions to the Loei Provincial Administrative Organization has been implemented in line with national decentralization policies. However, the process continues to face challenges, including insufficient budget allocation, shortages of personnel, and ineffective inter-agency coordination. Political roles in the area were found to exert both positive and negative influences. On the positive side, political actors contributed to advancing policies that addressed public needs promptly. On the negative side, political interferences such as inequitable budget distribution and unfair personnel transfers—created problems of transparency and good governance in public health administration. The empowerment of local authorities and organizational integration as key strategies for enhancing efficiency in public health management while mitigating the negative impacts of local politics. The study recommends the establishment of strong independent monitoring mechanisms, the promotion of citizen participation in decision-making, and the reinforcement of good governance within local administrative organizations to ensure effective and sustainable public health management in Loei Province.
