Development of a Health Care Model for Elderly by Community Participation, Bang Pahan Subdistrict, Bang Pahan District, Phra Nakhon Si Ayutthaya Province

Abstract
The purpose of this participatory action research article 1) study the situation and challenges in elderly healthcare 2) develop a community-based participatory elderly healthcare model, and 3) evaluate the effectiveness of the developed model. The study sample included 74 dependent elderly individuals and 54 stakeholders, totaling 128 participants selected through purposive sampling. Data were collected using questionnaires and focus group discussions, and analyzed using descriptive statistics, t-tests, and content analysis. The research results were found as follows; (1) 5.20% of the elderly were homebound, with most suffering from chronic non-communicable diseases, requiring caregivers, and being at risk of complications; (2) the community-based participatory elderly healthcare model comprised six key components: (1) network establishment, (2) capacity building for stakeholders, (3) caregiver training, (4) home visits for homebound and bedridden elderly, (5) support from mentor teams, and (6) knowledge exchange; and (3) after implementation, the elderly had significantly improved quality of life at a .05 significance level. Key success factors included: (1) a strong sub-district committee, (2) effective collaboration among multidisciplinary teams and relevant personnel, and (3) commitment and dedication of elderly caregivers.
