Nursing Care of a Schizophrenia Patient Due to Methamphetamine Abuse

Abstract

This study aims to examine the nursing outcomes of a patient diagnosed with schizophrenia and a history of methamphetamine use, utilizing the nursing process integrated with Peplau’s Theory of Interpersonal Relations as the conceptual framework. The case study focuses on a 29-year-old Thai male who voluntarily admitted himself to Rajavithi Hospital. The patient initially presented with irritability, agitation, confused speech, poor eye contact, and aggressive posturing during interviews. The preliminary medical diagnosis identified mental and behavioral disorders resulting from the use of central nervous system stimulants. The patient underwent comprehensive treatment and rehabilitation until September 5, 2022, during which nursing care was implemented based on seven prioritized nursing diagnoses. The clinical evaluation revealed significant therapeutic progress. The patient exhibited no aggressive behaviors toward himself or others and demonstrated an improved ability to manage thoughts and emotions appropriately. Furthermore, his self-esteem and self-acceptance notably increased. Regarding health literacy, the patient successfully articulated the detrimental effects of substance abuse and identified correct self-care strategies to prevent relapse. In terms of pharmacological intervention, no adverse side effects from psychiatric medications were observed. The patient remained compliant with his medication regimen, maintained daily functional activities, and established an effective sleep pattern, averaging 5–6 hours per night. Behavioral outcomes indicated high cooperation with the therapeutic program and positive behavioral modifications. Although occasional emotional dysregulation occurred, the patient showed rapid adaptation and a willingness to accept professional assistance. Finally, both the patient and his family demonstrated a thorough understanding of nursing discharge instructions, maintained a positive attitude toward continued treatment, and were prepared for home-based self-care. These findings highlight the efficacy of utilizing interpersonal relationship-based therapy in rehabilitating psychiatric patients with co-occurring substance use disorders, facilitating a successful transition toward recovery and long-term stability.
